Accident: near Lebuhanraio, Indonesia 03-Apr-1990. Soon after take-off en route, to Bali when the left engine quit following an engine overspeed, the aicraft lost height and hit a hill-top and bounced over the top. It came to rest in trees on the, other side. Three crew and 14 passengers survived. The aircraft was damaged beyond repair.
