Accident: Diyai mountain, Irian Jaya. 04-Nov-1994. Crashed in dense jungle and caught fire at the 7,500 foot level on Diyai mountain, Irian Jaya just before landing at Obana airport en route from Nabire to Kebu.  All 3 crew and 1 passenger killed. Aircraft written off.
