• FAE-457 de Havilland Canada Inc., Downsview, ON. Rolled out 04-Mar-1975. First flight 26-Mar-1975.

• FAE-457 Delivered to Ecuador Air Force/ Fuerza Aérea Equatoriana, Operated by Escuadrón Twin Otter, 24-Apr-1975.

• HC-BAX Transferred to TAME- Transportes Aéreos Militares Equatorianos (dual regn with FAE 457), 1977.

Accident: Putmayo 27-Jul-1979 Overran runway on take-off. Repaired.

• HC-BAX Returned to service with TAME, 02-Oct-1980.

Accident: Near Zumba 21-May-1981. was destroyed when it crashed into the side of a fog shrouded mountain. All 3 crew & 15 passengers on board were killed. Aircraft written off.

Note. Several reports indicate the plane crashed on May 21 or May 22 near Zumba, Ecuador. Other reports suggest that the Twin Otter crashed while flying in formation with the Presidential Beechcraft 200 on May 24, 1981. This accident happened near Macará, Ecuador, killing Ecuadorian President Jaime Roldós Aguilera aboard the Beech 200. Newspaper reported at the time do however not report the simultaneous accident of the Twin Otter. It is thus considered probable that the Twin Otter crashed in a separate accident a few days earlier.
