Accident Anggi, Papua, Indonesia. 22-Apr-1999. During take-off roll the aircraft departed the runway and collided with a drainage ditch. Repaired and returned to service.

Accident Frans Kaisiepo, Biak, Indonesia.22-Jan-2000. On rollout after landing the aircraft departed the left side of the runway before coming to a stop approx. 60 metres from the runway edge.  Repaired and returned to service.

Note: Merpati ceased operations in Feb-2014. The aircraft was withdrawn from service at Biak, Indonesia and noted in a derelict condition, mid 2015.
